Download Test Request Forms

Test Request Forms (formerly Chain of Custody forms) are required for all samples submitted to RSA Geolab. These forms ensure accurate sample identification, proper handling, and reliable testing results.
Select and download the appropriate form below based on your material type and testing requirements. Using the correct form helps prevent delays and ensures your samples are processed according to applicable standards.

Before Submitting Your Samples

Please confirm the following before submission:

  • All required fields are completed
  • Test methods (ASTM, AASHTO, etc.) are clearly specified
  • Sample identification matches submitted materials
  • Contact information is accurate

Incomplete or incorrect forms may delay processing.

How to Submit Samples

Download and complete the appropriate Test Request Form for your material type. Ensure all required fields are filled out and that sample labels match the form. Include the completed form with your shipment or submit it through the online upload option. Proper documentation helps prevent delays and allows testing to begin promptly upon receipt.

Additional Submission Guidance

Selecting the correct Test Request Form helps ensure your samples are routed to the appropriate testing process without delay. If multiple material types or test methods are required, separate forms may be needed to clearly define each request.

For larger or more complex projects, providing detailed project information and testing requirements can help streamline processing and improve turnaround times.

If you want a step-by-step overview of forms, uploads, and material-specific requirements, read our guide on: how to submit soil and geosynthetic samples.

What Is a Test Request Form?

A Test Request Form (TRF) is used to document the transfer, handling, and testing requirements of samples submitted to the laboratory, maintaining the chain of custody for each sample. It ensures that all materials are properly identified, tracked, and processed according to project specifications and testing standards.

The form includes important details such as sample identification, requested test methods, project information, and contact details. Providing complete and accurate information helps ensure that testing is performed correctly and that results are clearly associated with the submitted materials.
